What it is

The nicotine patch is a transdermal patch worn on the skin that releases a steady dose of nicotine over 16 or 24 hours, depending on the product.

How it works

It maintains a stable, low level of nicotine in the blood, which reduces withdrawal symptoms and cravings, and makes the habit easier to break. Dose is usually stepped down over weeks.

Evidence and effectiveness

Cochrane reviews consistently find that nicotine replacement therapy (including the patch) increases the chance of quitting by roughly 50–60% compared with placebo or no medication.

Guidelines: Recommended by WHO (2024), USPSTF (2021) and NICE NG209 (UK). NRT products were first prequalified by WHO in April 2024.

Compared with other options: Single-form NRT raises quit success by roughly 50–60% versus placebo. Combining the patch with a fast-acting form (gum or lozenge) is more effective than either alone and roughly comparable to varenicline.

Common adverse effects

Skin irritation at the application site is most common. Vivid dreams or sleep disturbance can occur; some users remove the patch overnight. Headache is also reported.

Important safety considerations

Widely available without prescription. People with recent heart attack, stroke or unstable angina should discuss use with a healthcare professional first. In pregnancy, behavioral support is preferred; any NRT use should be under professional guidance.
